As a member, you will have a dedicated Primary Care Physician (PCP) reviewing all aspects of your health and helping navigate the complex healthcare system.

Metabolic Health and Weight Management

As a member, you will have the support and guidance needed for achieving and maintaining a healthful weight.  You will have access to unlimited Seca body scans to track your progress.

Hormone Replacement Therapy

Your PCP will guide you through personalized plans to address hormonal changes or imbalances.  Direct communication with your physician aides in tailoring the approach.

Aging and Longevity

Detection and tracking of biomarkers is paramount in the pursuit of graceful aging.  As a member, your long term health is a focus of each and every visit.

We would love to answer all your questions about the practice and how it can help you achieve your health goals.  Before starting a membership, we suggest scheduling a free 15-min conversation with Dr. Fagan so that you can feel confident in your membership decision.

Utilizing 45 minute appointments, a deeper connection and understanding is made allowing you to be a part of your health and wellness.

Four Mile Creek Health is based on a membership model of healthcare.  You are billed a set rate monthly, so that there are no bills or invoices for services received.

All interactions, office visits and exams are covered by your monthly membership fee.  Your personal doctor is there to help you removing all billing incentives.
